Chronic Disease Management Care Plan

Service Description

To be eligible for a Care Plan, your GP must identify that you have a chronic medical condition that has been, or is likely to be, present for six months or longer. A chronic medical condition is one that has been or is likely to be present for at least six months, e.g. asthma, cancer, cardiovascular illness, diabetes mellitus, musculoskeletal conditions and stroke. You must have a referral from your GP which will entitle you to a rebate, reducing the cost of your appointment. Be sure to bring your medicare card to the appointment.
